Here at NSFHC we have 2 visiting Allied Health Professionals.

 

Diabetic Educator 

Our Diabetes Educator visits the practice 3 weekly on a Tuesday

Our educator has extensive experience with chronic disease management while helping empowered patients to implement effective diabetes self-management strategies.

This program is funded through Marathon Health for patients of this practice.

 

Dietitian 

Our Dietitian provides food and nutrition information, and supports people to improve their health as well as providing advice on nutrition-related matters. They can also change diets to help manage conditions such as:

•diabetes

•heart disease

•overweight and obesity

•food allergies and intolerances.

Our Dietitian visits the practice twice a month on a Tuesday.

This program is funded through Marathon Health for patients of this practice.
